  6. Latest Securities and Exchange Commission General Information Sheet (SEC GIS), Articles of Incorporation and By-Laws
  7. Board Resolution – if the signatory is not part of the Board of Directors
  8. Latest Income Tax Return (ITR) with corresponding proof of payment
  9. Latest Mayor’s Permit
