- Submission of the latest SEC Articles of Incorporation and By-Laws (CTC per page)
- Latest Mayor’s Permit – with received/stamped by the Municipal
- 2 pcs of 2”2 photos (white background without eyeglasses)
- Latest Income Tax Return with proof of payment
